The Caribou School Department Health Services office will be offering a tetanus, diphtheria and pertussis (Tdap) clinic in February. This clinic will be free to all children 11-18 years of age within the Caribou School System. Paper work for those students needing a Tdap immunization will be sent home with the child’s progress report. This paperwork will need to be returned by January 24.
If a child does not need the immunization because he/she has received it at a physician’s office, parents and guardians are asked to please bring in the student’s shot record to be copied or have a provider ‘s office fax it to 493-4254.
State law requires that the Caribou School Department’s Health Services view the actual written immunization record as provided by the health care provider. Information cannot be taken over the telephone.
