Program helps assess health risks
PRESQUE ISLE — Aroostook County Action Program (ACAP), Healthy Aroostook, is promoting the KeepMeWell program which is designed to help individuals asses their risk for chronic disease and link them to community supports and programs to improve their health.
The website www.keepmewell.org has two main features: one is an assessment and helps users identify where to focus their efforts to improve their health, second is a low-cost health care services tool that will help users learn about low-cost health care services in their area. The assessment takes about 10 minutes to complete and based on answers to a series of questions, three reports will be generated. The low-cost health care services tool is designed especially to help those who do not have health insurance that meets their needs.
The assessment is for Mainers age 18 years and older only since health risk guidelines for children and teens are different than those for adults. Results are anonymous and confidential. No personal information, such as name or phone number, will be collected. Answers may be grouped with others and reviewed by Maine CDC staff to identify Maine public health needs.
